Thank you for your application to the 成人大片 (成人大片) and Centennial College Nursing Collaborative Program. We think you鈥檝e made a great choice! If you鈥檝e submitted your application, our Admissions team will be in touch via email very soon.
In the meantime, follow the steps below to ensure that your documents are received and to stay up-to-date on the status of your application.
If applying through , the Collaborative Nursing Program supplementary form must be completed by all applicants who aren't currently enrolled in full-time studies at an Ontario secondary school.
If you aren't a current Ontario secondary school student, please complete all sections of the Collaborative Nursing Program supplementary form and email the completed form to Centennial College at admissions@centennialcollege.ca.
Be sure to monitor your email account for updates on the status of your application. You'll be invited to create a Choose成人大片 Applicant Portal where admission decisions are posted.
The majority of admission decisions are released between February and the end of May.
If you applied through and are selected for an offer or waitlist decision, you'll receive an email invitation from 成人大片 requesting you activate your Choose成人大片 Applicant Portal where this decision will be posted. All other decisions will be issued by Centennial College.
Please continue to visit your Choose成人大片 Applicant Portal for updates on the status of your application.
Subject to final confirmation in June, tuition and ancillary fees for full-time attendance can be viewed on the tuition and fees by program page.
Step 1: Double-check your application
飬� Did you forget anything?
- Have you listed every post-secondary school where you enrolled in one course or more? This is required regardless of its relevance to your application.
- Did you include a list of all your non-academic activities (e.g. employment, volunteer work, travel, etc.) from year to year? Your list must be from the end of secondary school to your expected start date. This isn't required for current Ontario secondary school students. If there鈥檚 a timeframe when you didn鈥檛 participate in any activities and didn鈥檛 attend school, you must state "no activities."
- Applying for readmission? You must include an outline of your activities since you were last enrolled in your program and submit updated academic transcripts (if applicable).
飱� Need to make changes to your application?
Do you need to make edits to your application, such as changing your program choices or contact information? These changes must be made as soon as possible to avoid missing any deadlines. Here鈥檚 how:
- If you applied via the Ontario Universities鈥� Application Centre (OUAC), make your changes in your .
- If you applied via ontariocolleges.ca for the Collaborative Nursing Program, make your changes in your .
- All other applicants must complete the 成人大片 change form.
Step 2: Activate your 成人大片 online identity
飥� You need access!
In order to access our secure student information system, activate your 成人大片 online identity by filling in the activation form. When asked for your Student/Applicant ID, enter your 9-digit 成人大片 Student Number (located in your application acknowledgement email). Your 成人大片 Online Identity will be activated within 24 hours.
Step 3: Log into your Choose成人大片 Applicant Portal

Your Choose成人大片 Applicant Portal is our primary way of communicating with you, so check your portal often. Track your application status, upload your documents, review your English language proficiency status and read your communications 鈥� all in one spot!
Admission communications are date sensitive. If a response is required, be sure to meet the deadlines in your letter.
Step 4: Submit your transcripts and supporting documents
飬� Have you submitted your documents?
We鈥檝e received your application, now we need your supporting documents so that we can begin our assessment.
- If you鈥檝e applied through , you should submit your documents directly to Centennial College.
- If you鈥檝e applied through the 成人大片 Application, visit the document submission page to determine what documents are required in support of your application, as well as for submission instructions.
- If you鈥榙 like to provide information about extenuating circumstances that have negatively impacted your academic performance, we encourage you to submit a special consideration form.
- Not sure if you need to provide an English language proficiency test score? Review your English proficiency result status bar in the Choose成人大片 Applicant Portal.
Remember: Admission decisions are based on the information available when we review your application. Don鈥檛 delay - submit your documents today!
Step 5: Maintain your email

Make sure important and time-sensitive emails aren鈥檛 blocked by spam filters. Add noreply@torontomu.ca to your email contact list to ensure you don't miss these emails.
